SLIDING FEE SCHEDULE

It is the policy of Trinity Family Health to ensure that the organization provides a sliding scale to health center patients who are at or below 200% of the Federal Poverty Level (FPL).

Trinity Family Health uses the United States Health and Human Services annual Federal Poverty Guidelines release to determine eligibility for our sliding fee discount program. The Federal Poverty Guidelines are released at the beginning of each calendar year and reviewed by the Trinity Family Health Board of Directors in the month following release.
